Job Description
- Provide scheduled onsite physician rounds at assigned facilities and manage acute and chronic conditions for medically complex older adults.
- Collaborate closely with Pinnacle Advanced Practice Providers and Care Coordinators to develop and oversee patient-centered plans of care.
- Support transitions of care, hospitalization follow-up, goals-of-care discussions, medication management, and efforts to prevent avoidable emergency department visits and hospitalizations.
- Participate in quality improvement, value-based care, population health, and care-gap closure initiatives.
- Maintain timely, accurate, and compliant medical documentation and coding.
- Build strong working relationships with facility Administrators, Directors of Nursing, and clinical teams.
- Participate in facility quality meetings and clinical initiatives as appropriate and potentially serve as Medical Director for one or more skilled nursing facilities. Qualifications
- MD or DO with an active, unrestricted medical license in the applicable state.
- Board certified or board eligible in an appropriate specialty and eligible to participate in Medicare and Medicaid.
- Strong clinical judgment, communication skills, and ability to work effectively within an interdisciplinary team.
- Experience in geriatrics, internal medicine, family medicine, skilled nursing, long-term care, or post-acute medicine preferred.
