COMPANY PROFILE
This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 50 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider's office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.
POSITION DESCRIPTION
The physician serves as the primary care provider for a dedicated panel of geriatric patients. Patients are located in long-term care facilities (ALFs / ILFs) and private homes (private home patients are optional). Physicians are assigned a convenient geographic area in which all patients are located. Physicians see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs, but typically see 40 to 60 patients per week. Dedicated schedulers put together daily schedules / routes for the providers to ensure efficiency and appropriate time for every visit. Specific duties of the role include, but are not limited to:
- conduct health assessments
- develop personalized care plans
- evaluate patient conditions and responses to treatment
- adjust treatment plans as necessary
- perform minor procedures and wound care as required and as settings permit
- educate patients and families on health management, disease prevention, etc.
- provide guidance on lifestyle modifications and self-care practices
- coordinate with nurse practitioners, nurses, and other healthcare professionals The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
On-call is phone triage only with very low volume and providers rotate the on-call responsibility, typically one (1) night per month.
REQUIREMENTS
- Ohio medical license
- DEA•Board Certified or Eligible, Internal / Family / Geriatric Medicine
