This role emphasizes preventive medicine, chronic diseasemanagement, and building meaningful, long-term patient relationships in asupportive outpatient environment. You will work within a collaborative, multidisciplinary care modeldesigned to improve patient outcomes, enhance access to care, and reduceunnecessary hospitalizations. Our approach prioritizes personalized care plans,proactive health management, and patient education. Key Responsibilities Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions. Deliver preventive care, wellness visits, and health screenings with a focus on early intervention. Develop and manage individualized care plans for a predominantly geriatric population. Collaborate closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care. Utilize electronic health records (EHR) to document clinical encounters and track patient outcomes. Engage in quality improvement initiatives and contribute to a culture focused on continuous care enhancement. Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices. Qualifications MD or DO from an accredited medical school Internal Medicine or Family Medicine or Geriatric Medicine Active, OK license Current DEA registration At least 3 years of experience in Outpatient Primary Care Strong clinical judgment with a focus on preventive and longitudinal care Excellent interpersonal and communication skills Commitment to a team-based, patient-first care model Why Join Us?
