Job Description

Position Summary:
We are seeking a motivated and compassionate Primary Care Advanced Practice Provider (NP or PA) with experience in rheumatology to join our team. The ideal candidate will provide primary care services while managing and coordinating care for patients with rheumatoid arthritis and related conditions.
Key Responsibilities:
Provide comprehensive primary care to adult patients, including preventative care, chronic disease management, and acute care visits. Assess, manage, and coordinate care for patients with rheumatoid arthritis and other autoimmune disorders. Collaborate closely with rheumatologists and other specialists to optimize patient outcomes. Educate patients and families on disease management, medication adherence, and lifestyle modifications. Maintain accurate and timely documentation in the electronic health record (EHR). Participate in quality improvement initiatives and continuing education activities.
Qualifications:
Certified Nurse Practitioner (NP) or Physician Assistant (PA) with an active state license. Minimum of 2 years of primary care experience; experience with rheumatoid arthritis or autoimmune conditions strongly preferred. Strong clinical assessment, diagnostic, and patient education skills. Ability to work collaboratively in a multidisciplinary team environment. Commitment to evidence-based practice and lifelong learning.
Pay:
$120,000.00 - $130,000.00 per year
