Family Medicine Physician - Advanced Primary Care | Eugene, Oregon Shaping the Future of Primary Care Advanced Primary Care (APC) - Eugene, part of Praxis Health, is seeking an experienced Family Medicine Physician to join our growing Eugene practice. APC is reimagining what primary care can look like through an innovative, value-based model focused on access, prevention, population health, and meaningful patient relationships. Rather than simply treating illness as it arises, our providers proactively manage chronic conditions, close care gaps, and partner with patients to help them stay healthy and out of the hospital. Practice Highlights Four-day clinical week with 32 patient-facing hours + 8 hours of dedicated administrative time Lower patient volume and thoughtful appointment lengths to deliver exceptional and individualized care Adult primary care population, ages 18+, with an emphasis on preventive care, chronic disease management, wellness, and population health Opportunity to provide a broad range of primary care procedures and women's health services Collaborative care environment with APP, behavioral health, clinical, operational, and population-health support Very light shared call Compensation & Benefits Base salary:$275,000 with a two-year guarantee Opportunity for productivity/quality compensation
