Job Summary CommunityCare Hazleton is seeking a Family Medicine or Med-Peds trained physician to care for patients of all ages at its growing primary care clinic in Hazle Township, PA. Job Duties Job Description CommunityCare, a Federally Qualified Health Center Look-Alike affiliated with Geisinger and qualifies for the National Health Service Corp Loan Repayment Program and the Pennsylvania Loan Repayment Program. In this role, you will partner with interdisciplinary teams to manage population health, improve outcomes, and drive preventive care initiatives. The patient panel includes comprehensive care across the full lifespan, from newborns to end-of-life care. Practice Overview and Schedule + 100% outpatient practice + 40-hour work week, including: + 36 patient contact hours + 4 administrative hours + Family Practice clinic hours (subject to change): + Monday
