$220,000 - $255,000 per year Job Details - We are seeking a dedicated and passionate Family Medicine Physician to join our robust healthcare team. This is a permanent, full-time role that offers an exciting opportunity to provide comprehensive care to patients of all ages. You will be working closely with a team of committed healthcare professionals to deliver exceptional patient care. The ideal candidate will be skilled in Evidence-Based Medicine, Chronic Disease Management, Preventive Care, Care Coordination, Medication Management / Prescribing, and Population Health.
Responsibilities:
Provide comprehensive and continuous care to patients and their families. Utilize evidence-based medicine to diagnose and treat a variety of acute and chronic conditions. Develop and implement patient management plans, including preventive care. Coordinate patient care with other healthcare professionals and refer patients to specialists when necessary. Prescribe and manage medications in accordance with patient needs and regulatory guidelines. Monitor the health status of patients with chronic conditions and adjust treatment plans as necessary. Educate patients and their families on disease prevention and health maintenance. Participate in population health initiatives to improve the health of the communities we serve. Adhere to all relevant medical laws, regulations, and standards.
Qualifications:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school. Board certified or board eligible in Family Medicine. A minimum of 2 years of experience in Evidence-Based Medicine, Chronic Disease Management, Preventive Care, Care Coordination, Medication Management / Prescribing, and Population Health. Valid state medical license and DEA license. Exceptional clinical, diagnostic, and communication skills. Strong commitment to patient-centered care and a passion for improving the health of our communities. Ability to work collaboratively with other healthcare professionals.
