A growing, physician-led post-acute care organization is seeking a dedicated Physician to provide primary care services within long-term care communities across Pennsylvania. This role offers a structured weekday schedule, strong interdisciplinary support, and the opportunity to care for a consistent patient population. What This Role Offers
  • Monday Friday daytime schedule
  • Minimal call requirements
  • No overnight hospital rounding
  • A consistent patient panel within LTC communities
  • Strong interdisciplinary collaboration
  • Administrative and documentation support While some visits are brief and focused, the overall model prioritizes continuity of care and long-term patient relationships particularly for medically complex seniors. Clinical Responsibilities
  • Conduct admissions and new patient evaluations
  • Manage chronic and acute medical conditions in the LTC setting
  • Perform routine follow-ups and regulatory visits
  • Collaborate with NPs, nursing staff, therapists, and facility leadership
  • Communicate with families and caregivers
  • Document encounters using a company-issued mobile device The focus is on quality, coordination, and patient-centered care within a structured post-acute environment. Schedule
  • Monday Friday
  • Daytime hours
  • Minimal on-call rotation

  • Base salary starting at $250,000+
