Primary Care Physician Opportunity Central Valley Community Health Network California Join a mission-driven, multi-county Federally Qualified Health Network serving diverse communities across Kern County , Kings County , Tulare County , and Fresno County . Practice in modern, state-of-the-art health centers offering fully integrated, patient-centered care across a wide range of specialties, including medical, women s health, dental, behavioral health, pharmacy, chiropractic, optometry, and specialty services. Position Highlights 100% outpatient primary care Patient-Centered Medical Home model Monday Friday, 9:00 AM 5:00 PM schedule Option for 4/10 work schedule or part-time flexibility Light call: 1 week per quarter (from home)
