Physician Liaison Purelife Home Health Duarte, CA Job Details Part-time | Full-time | Contract $1,500 - $2,500 a month 6 hours ago Benefits Flexible schedule Qualifications Customer follow-ups Data analysis skills Data reporting Client base expansion experience Sales territory planning Client retention strategy Sales follow-up Full Job Description Community Liaison / Business Development Representative - Medicare-Certified Home Health Agency We are a Medicare-certified home health agency seeking a part-time Community Liaison to build professional relationships with hospitals, discharge planners, skilled nursing facilities, primary care offices, assisted living communities, and other community healthcare partners.
Responsibilities include:
Conducting compliant outreach to referral sources and community partners Educating providers and discharge teams about our services, coverage area, accepted payers, intake process, and clinical capabilities Scheduling and conducting in-service presentations Maintaining accurate documentation of outreach activity, contacts, and follow-up items Helping improve referral workflows and communication with providers Representing the agency professionally and ethically in accordance with Medicare, anti-kickback, HIPAA, and agency compliance standards
Requirements:
Prior experience in home health, hospice, SNF, hospital discharge planning, physician relations, healthcare marketing, or case management preferred Existing professional relationships with discharge planners, PCP offices, SNFs, or senior-care communities strongly preferred Strong communication, organization, and follow-up skills Must understand that compensation is for legitimate marketing and outreach services only and is not based on patient referrals, admissions, revenue, or volume/value of business generated
Compensation:
Hourly or fixed monthly compensation based on experience and agreed-upon outreach responsibilities. No per-referral or per-admission compensation.
Pay:
$1,500.00 - $2,500.00 per month
