About InstaMD InstaMD partners with physician practices to support patients through Remote Patient Monitoring (RPM) and chronic care services. We help patients monitor their health at home, stay connected with their care teams, and receive timely support between office visits. About the Role We are seeking a compassionate, organized Certified Medical Assistant to support our RPM program remotely. You will help patients get started with home monitoring devices, encourage consistent readings, review incoming information according to established protocols, and communicate patient concerns to the supervising clinical team. This position involves frequent patient calls, accurate documentation, and coordination with nurses, physicians, and practice staff. All clinical activities must remain within the CMA's authorized scope, training, and assigned supervision. Key Responsibilities Contact patients to explain the RPM program and support enrollment, documenting consent according to practice procedures. Educate patients on using connected blood pressure monitors, weight scales, glucose meters, and pulse oximeters, as applicable. Assist with device setup, basic troubleshooting, and coordination of shipping, replacements, or returns. Review incoming readings and assigned alerts according to approved protocols; promptly route abnormal readings or reported symptoms to the supervising nurse or provider. Follow up with patients who have missing readings, difficulty using devices, or barriers to participation. Gather and document patient-reported symptoms, medication information, and adherence concerns for clinical review. Reinforce care team instructions and approved patient education materials. Maintain accurate, timely records of calls, readings, patient concerns, actions taken, and time spent on eligible activities. Coordinate with physician practices and internal teams to track patient needs and close outstanding follow-up tasks. Protect patient privacy and follow HIPAA, information security, and company policies. Meet established expectations for patient outreach, documentation quality, responsiveness, and service. Required Qualifications Current Certified Medical Assistant credential; indicate the issuing organization when applying. At least one year of medical assisting or direct patient care experience. Familiarity with vital signs, medical terminology, and common chronic conditions. Strong telephone communication skills and a patient, empathetic approach. Experience documenting patient interactions in an EHR or clinical platform. Ability to recognize concerns that require escalation and follow established workflows. Strong organizational skills and the ability to manage an assigned patient panel independently. A private, quiet workspace, reliable internet connection, and availability during scheduled working hours. Preferred Qualifications Experience in RPM, chronic care management, telehealth, or patient outreach. Experience supporting patients with hypertension, diabetes, chronic kidney disease, or heart failure. Experience working with older adults and patients who need additional technology support. Familiarity with Epic, eClinicalWorks, Practice Fusion, Cerner, or similar EHR systems. Bilingual English/Spanish proficiency.
