Manage adult patient panels, providing personalized chronic, acute, and preventive care. Deliver comprehensive care, conduct screenings, vaccinations, and promote wellness strategies. Utilize data for population health and care gap identification, supporting value-based care. Collaborate with interdisciplinary teams for coordinated, high-quality services. Maintain accurate EHR documentation, drive practice innovation, and achieve performance goals. Qualifications include MD/DO, active license, and experience in adult primary care or related fields.
