Role:

As a Post-Acute Care Physician, you will provide care for a panel of patients in skilled nursing and rehabilitation facilities. In this role, you'll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.

Core Responsibilities:

Provide in-person medical care to patients in designated post-acute facilities Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR Collaborate with facility teams to support quality outcomes and smooth transitions of care Collaborate as applicable in Trumen's value-based care and transitional care programs Develop strong working relationships with facility and clinical staff Serve as a liaison between Trumen and facility teams Maintain an on-site presence consistent with your agreed schedule and facility needs

Qualifications:

MD or DO with a current, unrestricted Texas medical license Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine 1+ year of experience in post-acute, geriatric, or SNF care preferred Comfortable practicing independently in skilled nursing facilities Strong clinical judgment, communication skills, and a patient-centered mindset Reliable transportation to assigned facilities About Trumen Physicians +
