Quest Homes is seeking a dependable, organized, and compassionate Care Coordinator to assist with coordinating services for individuals receiving Home and Community-Based Services (HCS). The Care Coordinator plays an important role in ensuring that individuals receive appropriate, person-centered services in accordance with their Individual Plan of Care (IPC), implementation plans, agency policies, and applicable Texas HCS requirements. This position works closely with individuals, families/LARs, direct care staff, nursing staff, program management, service coordinators, healthcare providers, and other members of the interdisciplinary team. The Care Coordinator is responsible for helping ensure services are properly scheduled, delivered, documented, monitored, and communicated.ESSENTIAL JOB DUTIES
Individual Care & Service Coordination Coordinate and monitor services provided to assigned individuals. Become familiar with each individual's IPC, implementation plans, identified needs, goals, preferences, and service requirements. Help ensure services are delivered according to the individual's authorized plan. Maintain regular communication with individuals, families/LARs, staff, nurses, supervisors, and other members of the service team. Assist with identifying unmet needs, changes in condition, service concerns, and barriers to successful service delivery. Promote individual choice, dignity, privacy, independence, community participation, and person-centered care. Appointments & Healthcare Coordination Coordinate medical, dental, therapy, behavioral health, specialist, and other necessary appointments. Ensure appointments are appropriately communicated to staff and transportation is arranged when applicable. Track upcoming and completed appointments. Obtain and review appointment summaries and provider instructions. Ensure new physician orders, medication changes, referrals, follow-up appointments, and recommendations are promptly routed to the appropriate nurse or supervisor. Maintain appointment and follow-up tracking systems. When assigned, transport individuals to medical, dental, therapy, behavioral health, specialist, or other appointments using a personal vehicle. Transport individuals to or from their homes, service locations, appointments, or other approved destinations as needed. Field Work & Travel Field work and occasional travel are required as part of this position. Visit individuals in their homes, residential settings, community locations, medical offices, and other service locations as assigned. Travel between Quest Homes locations, individual residences, appointments, meetings, and community settings. Use a personal vehicle for authorized work-related transportation and travel. Maintain a valid driver's license, current vehicle registration, required insurance coverage, and a safe, reliable personal vehicle. Follow all Quest Homes transportation, safety, confidentiality, and documentation policies when transporting or visiting individuals. Mileage reimbursement, if applicable, will be provided according to Quest Homes policy. On-Call Responsibilities Participate in periodic on-call coverage as scheduled by management. Remain available by telephone or other approved communication method during assigned on-call periods. Respond appropriately to urgent service, staffing, health, safety, transportation, or individual-related concerns. Follow agency procedures for escalating emergencies, incidents, medical concerns, abuse, neglect, exploitation, medication issues, and other significant events. Document on-call communications, actions taken, and required follow-up. Documentation & Records Review required service documentation for completeness and timely submission. Monitor service delivery logs, progress notes, daily documentation, appointment records, and other assigned records. Identify missing, incomplete, inconsistent, or late documentation and follow up with appropriate staff. Assist with maintaining organized individual records and program documentation. Protect confidential information and follow HIPAA and agency confidentiality requirements. Service Delivery & Billing Support Monitor authorized services and service delivery documentation. Assist with verifying that documentation supports services reported for billing. Track service units and identify potential missing documentation or discrepancies. Communicate documentation or service-delivery concerns to management before billing deadlines. Assist with monthly reconciliation of service records as assigned. Staff Coordination Communicate individual-specific needs and scheduling requirements to assigned staff. Assist supervisors with ensuring staff have access to current plans and instructions necessary to provide services. Follow up regarding missed services, staffing concerns, call-ins, scheduling issues, and documentation deficiencies. Immediately report concerns regarding health, safety, abuse, neglect, exploitation, medication issues, or other significant incidents according to agency policy. Nursing & Medication Coordination Work closely with the RN regarding nursing assessments, health changes, physician orders, medication changes, appointments, and staff nursing needs. Forward medication and health-related concerns to the RN promptly. Help track required nursing follow-ups and documentation. The Care Coordinator does not independently perform nursing assessments, nursing delegation, or other functions requiring a licensed nurse unless the employee separately holds the appropriate license and is assigned those duties by Quest Homes. Individual & Family Communication Maintain professional communication with individuals and their families/LARs. Respond to routine questions and appropriately escalate clinical, administrative, or service concerns. Participate in service-planning meetings, interdisciplinary meetings, staff meetings, and case reviews as assigned. Document significant communications and follow-up actions. Quality Assurance & Compliance Conduct routine reviews of assigned individual records. Help prepare records for internal audits, surveys, reviews, and monitoring activities. Monitor required documentation and follow-up deadlines. Immediately notify management of potential compliance concerns. Assist with corrective-action follow-up when assigned.DAILY RESPONSIBILITIES
The Care Coordinator should routinely: ☐ Review staff communications and individual-related concerns. ☐ Review upcoming appointments and transportation needs. ☐ Follow up on physician orders and appointment recommendations. ☐ Review documentation requiring correction or completion. ☐ Monitor significant health, behavioral, or service changes. ☐ Communicate with nursing and management as necessary. ☐ Address staffing or service-delivery concerns. ☐ Document important communications and follow-up activities. ☐ Report incidents and health/safety concerns immediately.WEEKLY RESPONSIBILITIES
☐ Review upcoming individual appointments. ☐ Follow up on outstanding appointment documentation. ☐ Review service-delivery documentation for assigned individuals. ☐ Identify missing or incomplete documentation. ☐ Review outstanding physician orders and referrals. ☐ Communicate unresolved concerns to management/RN. ☐ Review individual care and service needs with appropriate staff. ☐ Verify required follow-up activities have been completed. ☐ Complete assigned field visits and home or residential visits. ☐ Review upcoming on-call assignments and ensure availability as scheduled.MONTHLY RESPONSIBILITIES
☐ Review assigned individual records for completeness. ☐ Reconcile service-delivery documentation as assigned. ☐ Review service units and documentation for potential discrepancies. ☐ Verify appointment records and follow-up documentation. ☐ Review changes in individual needs, health status, behavior, or services. ☐ Assist with monthly quality-assurance reviews. ☐ Prepare documentation for administrative review and billing processes. ☐ Report recurring staff/documentation issues to management. ☐ Submit required mileage, transportation, field-visit, and on-call documentation.MINIMUM QUALIFICATIONS
Applicants should possess: High school diploma or GED required; associate or bachelor's degree in healthcare, social services, human services, business administration, or related field preferred. Experience inHCS, IDD
services, healthcare, case management, residential services, or community-based services strongly preferred. Strong organizational and time-management skills. Ability to manage multiple individuals, appointments, deadlines, and documentation requirements. Strong written and verbal communication skills. Basic computer proficiency and ability to learn electronic documentation systems. Ability to maintain confidential information. Professional and dependable work habits. Ability to communicate effectively with individuals with intellectual and developmental disabilities, families, staff, healthcare professionals, and community partners. Valid driver's license, reliable personal vehicle, current vehicle registration, and required automobile insurance. Ability and willingness to use a personal vehicle for authorized work-related travel and transportation of individuals. Ability and willingness to perform field work, travel occasionally, conduct home or residential visits, and transport individuals as assigned. Ability and willingness to participate in periodic on-call coverage. Ability to successfully complete required background checks and employment requirements.PREFERRED EXPERIENCE

Annual Salary:
$33,000-$34,320 Final starting salary within the posted range may be based on qualifications, applicable experience, and job responsibilities.PAID TIME OFF & HOLIDAY PAY
