HarmonyCares, a physician-led, home-based primary care and palliative care organization, seeks a Physician Leader in Springfield, VA. In this role, you will provide direct in-home and telehealth primary care to medically complex, home-limited adults while leading an interdisciplinary team of NPs, nurses, social workers, and care coordinators. You will oversee clinical quality, support value-based care initiatives, and reduce avoidable hospitalizations through proactive care planning. Responsibilities Provide home-based and telehealth primary care to medically complex, home-limited patients Lead and mentor an interdisciplinary team including NPs, nurses, social workers, and care coordinators Develop and oversee individualized care plans to reduce avoidable ED visits and hospitalizations Monitor and improve clinical quality, patient safety, and satisfaction metrics Collaborate with operations and case management to coordinate in-home visits and virtual care Participate in value-based care initiatives, population health management, and performance improvement Ensure compliance with regulatory, documentation, and billing standards Engage patients and families in goals-of-care discussions, including palliative care when appropriate Use EHR and data analytics to track outcomes and drive evidence-based decisions Contribute to organizational strategy, culture, and physician leadership development at Harmony Cares.
