We are seeking a Social Services Designee to join our skilled nursing facility. This position plays an important role in supporting the social, emotional, and psychosocial needs of our residents and assisting residents and families throughout their stay at the facility. The ideal candidate is organized, compassionate, and comfortable working directly with residents, families, healthcare professionals, and facility staff. Long-term care experience is strongly preferred. Responsibilities Assist with the facility's admissions process, including communication with prospective residents, families, hospitals, and referral sources. Support new residents and families throughout the admission and transition process. Assist in identifying and addressing residents' social, emotional, and psychosocial needs. Maintain regular communication with residents and families regarding concerns, needs, and services. Work closely with nursing, therapy, admissions, administration, and other departments to coordinate resident care and services. Assist with discharge planning and coordination of services as needed. Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. Respond to resident and family concerns in a professional and timely manner. Participate in care planning and interdisciplinary meetings as appropriate. Ensure compliance with applicable federal, state, and local regulations and facility policies. Maintain resident confidentiality and professional standards at all times. Perform other related duties as assigned. Qualifications High school diploma or equivalent required; Associate degree preferred. At least 2 years of long-term care experience in nursing, activities, social services, admissions, or another resident-facing position preferred. Strong communication and interpersonal skills. Ability to communicate effectively and professionally with residents, families, staff, and outside organizations. Comfortable working in a fast-paced skilled nursing environment. Strong organizational and follow-up skills. Basic computer proficiency and ability to learn facility software and documentation systems. Ability to work independently while functioning effectively as part of an interdisciplinary team. Work Environment Skilled Nursing Facility Full-time or part-time opportunities may be available. Position involves regular interaction with residents, families, staff, healthcare professionals, and outside agencies. May require occasional weekend or extended-hour availability based on facility needs.
