Cornerstone Leader (Memory Care) Make Every Day Meaningful for Our Memory Care Residents The Summit, a Lifespire of Virginia Community, Is Looking for a Compassionate, Organized, and Inspiring Cornerstone Leader to Guide Our Memory Support Neighborhood. If You Have a Heart for Serving Seniors and the Leadership Skills to Bring a Person-centered Care Program to Life, We Want to Meet You. As Our Cornerstone Leader, You'll Serve As the Driving Force Behind the Cornerstone Memory Support Program. Responsibilities Champion the Cornerstone Program's Guiding Principles Across the Memory Care Neighborhood Guide Families Through the Lifestory Process and Use It to Shape Personalized Care and Programming Help Hire, Train, and Mentor Team Members Serving in Memory Care Partner with Nursing, Therapy, Culinary, and Activities Staff to Build a Program That Meets Residents at Every Stage of Dementia and Alzheimer's Disease Lead the Move-in Experience from Pre-admission Through the First Days in Their New Home Facilitate Monthly Care Meetings with Families and the Interdisciplinary Team Conduct Regular Environmental Rounds to Keep the Neighborhood Safe, Comfortable, and Full of Purposeful Moments Ensure Compliance with State and Federal Regulations and Community Policies Build Partnerships with Outside Organizations That Enrich Resident Experiences Qualifications A Degree or Relevant Experience in Memory Care- Nursing (Lpn/Rn), Recreational or Occupational Therapy, Social Work, Behavioral Health, or a Related Bachelor's Degree 2+ Years of Experience Working with Residents with Cognitive Impairment, Ideally in a Leadership Role Strong Understanding of Dementia and Its Impact on Residents' Abilities, Along with Strategies That Support Independence Excellent Organizational, Communication, and Problem-solving Skills A Warm, Steady Leadership Style- Someone Who Can Smile, Show Love, and Bring out the Best in a Team Certified Dementia Practitioner Certification (or Willingness to Obtain Within 3 Months of Hire) Why the Summit At the Summit, You'll Join a Community Rooted in Lifespire of Virginia's Mission and Values, Where Your Work Directly Shapes the Well-being and Dignity of the Residents Who Call Our Memory Care Neighborhood Home.
