Job Description

We are looking for an Activity Aide at our 20 bed, Grafton Assisted Living facility! This is a part-time job, afternoons during the week, and either each Saturday or every other weekend. You will be planning one activity each afternoon, or two on the weekend to lead the group! There is another activity coordinator to organize with. There is potential to pick-up more hours for activities +/or caregiving. Pay range is based on experience and prior certifications.
General Responsibility:
The Activity Aide implements a program of activities and events at Anita's Gardens of Grafton CBRF. The activities developed are to encourage independence, enhance their self-esteem, recognize and celebrate life accomplishments. In addition, activities should provide exercise, relaxation and fulfill basic psychological, social, and spiritual needs for our residents. The duties and responsibilities listed below are a general representation of what is expected of this position. Duties and responsibilities are subject to change based on communities needs and/or deemed necessary by management.
Daily Duties:
1. Implements group, as well as, one on one activities for those that chose not to participate in group activities. Encourages residents to engage in active participation. 2. Works with Activity Director to obtain supplies for activities, while staying within the company budget. Log and keep track of budget and receipts. 3. Prior to activity, invites and assists with transporting residents to and from activity areas using proper transfer techniques. 4. Recognizes and reports to manager/nurse any abnormal signs and symptoms which signify a physical or mental change in a resident's condition, and/or any emergency needs of the resident. 5. Drive community vehicle in order to transport residents to and from scheduled activity. 6. Manage the supervision of volunteers, students and entertainment groups. 7. Is motivated and enthusiastic when working with residents. Encourages residents to help plan and prepare for activities. 8. Responsible for assisting Activity Director with all necessary planning and preparation for birthdays, holidays and any special events. 9. Encourages residents to participate in religious activities to fulfill their basic spiritual needs, through visits with local ministers, attendance at religious services whenever possible, spiritual reading material. 10. Is responsible for maintaining the housekeeping of activity work areas and storage areas. 11. Will contact relatives at residents' request and may act as liaison with relatives. 12. Provide a safe environment by minimizing health and safety risks for residents throughout the entire building. 13. Assist the Activity Director with accurate record keeping of all services in the resident's activity log, progress notes, care plan log and other occurrences or issues such as; behavior tracking log, incident report, food log, etc. 14. Answer phone professionally and relay proper message to resident, employee or other contact. This includes writing the date, taking a name, phone number and a message of what call was regarding. 15. Provide a home-like atmosphere for all residents. 16. Understand and follow community's fire and disaster plans. Required to assist residents in emergencies; fire, tornado, drills, etc. 17. Required to call the appropriate department in cases of emergency; 911, EMTs, fire department, local police department, management, nurse. 18. Required to abstain from breaks during any activities. 19. Required to attend all in-services, meeting and training. 20. Maintain resident confidentiality per HIPAA requirements.
Other Requirements:
1. Extended hours and irregular shifts may be required especially in situations that cannot be avoided such as; severe weather, employee injury, etc., may occasionally be asked to help on the floor in emergency situations. 2. Expected to frequently walk, stand, bend, lift, reach, communicate, listen etc. 3. Occasionally stoop, kneel, crouch or crawl. 4. Ability to lift and/or move up to 35 pounds. 5. Follow DHS guidelines and Anita's Gardens policies and procedures. 6. Employees may be exposed to hazardous materials and infectious diseases. In these situations employees will be required to wear gloves, gowns and/or masks.
Qualifications:
1. Ability to exercise independent judgement and work independently. 2. Ability to operate various types of medical equipment as assigned. 3. Ability to maintain good relationships with individuals of varying social and cultural backgrounds. 4. Must be sensitive, compassionate and caring to the special needs of the residents. 5. Must have reliable transportation. 6.

Must maintain an acceptable work attendance. 8. Must be reliable and prompt when reporting to work.
