$2350-$2680/ week Job Summary We are seeking a dedicated and highly skilled Registered Nurse (RN) specializing in Long Term Care (LTC) and Skilled Nursing Facilities (SNF). The ideal candidate will possess extensive clinical expertise, a compassionate approach to patient care, and proficiency in managing complex health needs within a diverse resident population. As an RN LTC/SNF, you will be responsible for delivering comprehensive nursing care, coordinating interdisciplinary services, and ensuring compliance with regulatory standards to promote optimal health outcomes in a supportive environment. Duties Provide direct patient care, including assessments, medication administration, wound care, and vital sign monitoring in accordance with established protocols. Develop and implement individualized care plans tailored to residents' medical conditions, including memory care, hospice & palliative medicine, and behavioral health needs. Collaborate with physicians, therapists, and other healthcare professionals to coordinate treatment plans and ensure continuity of care. Utilize electronic health record (EHR) systems such as Cerner or Epic to document patient information accurately and efficiently. Conduct patient triage to identify urgent needs and prioritize interventions promptly. Administer specialized treatments such as botulinum toxin therapy or ostomy care when applicable. Ensure compliance with HIPAA regulations and maintain confidentiality of all patient records and medical documentation. Participate in discharge planning, staff education, and quality improvement initiatives aligned with NCQA standards. Skills Extensive experience working with individuals with disabilities, including developmental disabilities and dementia care. Proficiency in using EMR/EHR systems such as Cerner or Epic for documentation and order entry. Knowledge of advanced nursing procedures including IV infusion, catheterization, tube feeding, and aseptic technique. Strong understanding of medical terminology, ICD coding (ICD-9/ICD-10), and laboratory procedures. Critical care experience in settings such as ICU or trauma centers is highly desirable. Ability to perform comprehensive patient assessments utilizing physiology knowledge and clinical judgment. Excellent communication skills for effective collaboration with interdisciplinary teams and family members. Familiarity with Medicare/Medicaid regulations, MDS (Minimum Data Set), and long-term care standards. Experience managing acute pain, infectious disease care, trauma medicine, or emergency response procedures is advantageous. Pay:
$2,600.00 - $2,680.00 per week
