Registered Nurse - FT (13708) Hot Job Cullman Regional Medical Center - Cullman, AL 35056Apply Overview Position Type Full Time Job Shift Day Category Nurse Apply Description Job Summary:
Evaluate and admit patients to home care; develop plan of care and provide care to patient according to plan. Act as case manager making necessary adjustments in the patient's plan of care related to symptom changes and changes in care needs. Review and plan patient care and or discharge during scheduled case conference meeting and document accordingly. Act as liaison for patient/caregiver with MD and community healthcare partners. Strive to lower cost of care through managing mileage, supplies and visit scheduling. Completes all documentation related to patient contact, assessments, SOC, and recerts. Resumption of care and discharge according to policy. Maintain understanding of current home care regulatory guidelines. Provide on-call coverage for both home care and hospice as assigned. Demonstrate and encourage team behavior and exceptional patient/guest experiences. Uphold and promote patient safety and quality.Qualifications Education:
Associate's degree in nursing and currently licensed by the state of Alabama required.Experience:
Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER. Previous home care or hospice experience preferred.Additional Skills/Abilities:
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize a schedule to meet patient needs. Share job details toBenefits
