Job Description

Contribute your clinical expertise in a field-based home health nursing role, supporting patients in their homes by delivering compassionate and skilled care for a 13-week, full-time contract assignment. As a Registered Nurse specializing in home health, you will be vital in facilitating patient recovery, fostering independence, and collaborating with a multidisciplinary team using leading technology and proven assessment tools.
Schedule:
Monday Friday, 8:00 AM 5:00
PM Key Qualifications:
Active RN license in the state of practice, in good standing At least 1 year of recent home health nursing experience (field experience preferred) Proficient with Home Care Home Base (HCHB) electronic documentation Demonstrated experience with OASIS assessments Reliable personal transportation, valid drivers license, and current auto insurance
Primary Responsibilities:
Conduct comprehensive OASIS assessments and develop personalized care plans Provide skilled clinical interventions, including wound care, medication administration, IV therapy, and ongoing chronic disease management Evaluate patient conditions, monitor progress, and keep care teams and physicians informed of any changes Educate patients and families on disease management, medication adherence, and preventive care Complete documentation accurately and promptly using HCHB Collaborate with therapists, social workers, and home health aides for coordinated interdisciplinary care Participate in case conferences, staff meetings, and required trainings Consistently uphold all regulatory and documentation standards
