We partner closely with hospitals, physicians, skilled nursing facilities, and community organizations to support patients and families through every stage of care. We are seeking an experienced, relationship-driven marketer to expand our presence and referral base throughout Baldwin County. Position Summary The Home Health & Hospice Marketer is responsible for generating new patient referrals and strengthening existing relationships with referral sources across Baldwin County. This role focuses on hospital discharge planning, physician offices, skilled nursing facilities, assisted living communities, and community outreach. Success is measured by consistent census growth, strong referral partner satisfaction, and accurate representation of our clinical capabilities. Key Responsibilities Build and maintain strong referral relationships with hospital case managers, discharge planners, physicians, SNFs, ALFs, and other post-acute providers in Baldwin County. Conduct regular in-person visits, educational in-services, and lunch-and-learns to promote our home health and hospice services. Identify and pursue new referral opportunities while protecting and growing existing accounts. Accurately represent clinical capabilities, service lines, and admission criteria to referral partners. Collaborate closely with clinical leadership and intake teams to ensure smooth transitions of care and high referral conversion rates. Track referral activity, conversion rates, and competitor activity; provide regular updates to leadership. Participate in community events, health fairs, and professional networking opportunities that increase brand visibility. Maintain compliance with all applicable marketing, anti-kickback, and HIPAA regulations. Qualifications Proven experience in healthcare marketing, business development, or sales within home health, hospice, or post-acute care preferred. Existing relationships with Baldwin County hospitals, physician groups, or skilled nursing facilities strongly preferred. Self-motivated, organized, and comfortable working independently in the field. Excellent verbal and written communication skills; professional presence. Valid driver's license and reliable transportation required (significant local travel within Baldwin County). Knowledge of Medicare home health and hospice regulations a plus.
