We strive to meet and exceed the needs of those we serve by providing compassionate quality care. Summary We are seeking a Registered Nurse Case Manager to join our team at ExpectCare Hospice. In this role, you will play a vital part in coordinating patient care and ensuring that our patients receive the highest quality of service. Your contributions will directly impact the lives of patients and their families during critical times. Responsibilities Assumes primary responsibility for assigned patients and performs all phases of the nursing process. Communicates with attending physicians, ExpectCare Hospice staff members, and other agencies as necessary to coordinate optimal care and use of resources for patients/caregivers. Educates patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary. Documents all patient related information according to ExpectCare Hospice's policies and procedures. Document accurate and ongoing assessment of patient status via EMR System and ensure patient chart is up to date. Document patient care reflecting nursing interventions, patient response to care, patient needs, problems, capabilities, limitations, and progress toward goals. Seeks input from IDT members regarding the patient POC to obtain additional knowledge and support. Reviews and updates the POC at least every 60 days per regulatory standards. Regularly attends and participates in IDT meetings to ensure that the POC reflects current patient care needs. Assesses need for Certified Nursing Assistant (CNA) services. Conducts CNA supervisory visits per regulatory standards. Asses the need for and revises the CNA care plan. Informs the Clinical Director of unusual or potentially problematic patient/caregiver issues as identified. Assists other RNCM's with their patient assignments as needs are identified. Provides on-call coverage as scheduled. Maintains annual performance reviews and clinical competencies. Assist with patient admissions as directed. Identifies patients who may potentially revoke or need to be discharged. Demonstrate prompt and regular attendance. Exhibits cost effective use of time, mileage and supplies. Demonstrates familiarity with and adheres to ExpectCare Hospice policies and procedures, state, federal, and accrediting agency's requirements. Provides notification of absences according to ExpectCare Hospice policy. Promotes and maintains respect for rights, privacy, and confidentiality of others. Respects patient and staff confidentiality always. Attend staff meetings, mandatory in-services, and other meetings as appropriate or upon request. Participates in committees, task forces and ExpectCare Hospice events as requested. Assumes responsibility for own personal and professional growth including state licensing/certification requirements. Understands and exemplifies hospice philosophy, values, beliefs and team concepts in all job-related roles. Abides by dress code. Establish HHA plan of care supervising the Plan of Care per regulations. Performs other duties as assigned consistent with skills and training and the mission and goals of ExpectCare Hospice as required by Clinical Director. Requirements Current Registered Nurse (RN) license in the state of practice. Proven experience in case management or a similar role within healthcare settings. Strong critical thinking skills with the ability to assess complex situations effectively. Excellent communication skills for interacting with patients, families, and healthcare professionals. Previous hospice experience preferred. Knowledge of HIPAA regulations and medical terminology is essential.
