ASAP Key Responsibilities:

Perform comprehensive home health patient assessments in stable and emergent situations. Develop, implement, update, and evaluate individualized Plan of Care (POC). Provide skilled nursing care, medication administration, wound care, IV therapy, catheter care, and phlebotomy services. Complete accurate OASIS documentation and maintain timely clinical records. Educate patients and families on medications, disease management, home safety, and self-care practices. Coordinate care with physicians, therapists, case managers, and interdisciplinary healthcare teams. Monitor patient progress and ensure compliance with payer and regulatory requirements. Assess and manage pain using appropriate interventions and patient education. Supervise Home Health Aides and support staff to ensure continuity and quality of care. Participate in on-call rotation and provide after-hours patient support as assigned. Maintain infection control and OSHA safety standards during all patient visits. Attend interdisciplinary team meetings and participate in quality improvement initiatives.

Qualifications:

Active Registered Nurse (RN) license required. Minimum 1-2 years of RN clinical experience required. Minimum 2 years of Home Health experience required. Homecare Homebase (HCHB) experience required. OASIS documentation experience required. Current CPR certification required. Valid driver's license, reliable transportation, and auto insurance required. Strong knowledge of wound care, IV therapy, catheter care, and home health nursing practices. CHPN certification preferred. BSN preferred.

Pay:

$2,000.00 - $2,200.00 per week
