In this position, the Licensed Practical Nurse will often interact with Staff Members, Residents and their families, other department managers, vendors, and other professionals and should therefore be able to communicate effectively.
Responsibilities:
The following responsibilities have been incorporated into this position. This list is not necessarily all-inclusive and other special tasks and responsibilities may be assigned as necessary. Focus on the assessment and treatment of patient wounds. Assessing complex patient wounds on residents. Cleaning resident wounds to remove bacteria and debris and minimize the risk of infection Dressing resident wounds in accordance with nursing practice and physicians instructions Determine appropriate treatment to assist in wounds healing Teaching patients and caregivers how to treat and dress wounds to assist healing and prevent infection after discharge. Giving verbal instruction and creating a patient care plan Administers prescribed medications and treatments in accordance with approved nursing techniques; annotates time and amount on patient charts. Knowledge of common action, usual dosage and side effects Responsible for documentation of patient condition; records in the clinical record and reports all observed symptoms, reactions, treatments and changes in the patient's condition. Observes patient's conditions and reaction to drugs, treatments and significant incidents Takes resident's vital signs to detect deviations from normal and assesses condition of resident Maintains compliance to all personnel policies, established community policies and procedures, Federal and State regulations and standards, including, but not limited to HIPAA privacy and confidentiality laws; understands and implements the Fire Safety and Disaster Preparedness Plan and evacuation procedures of the community during drills and actual emergencies. Documents services performed for billing purposes Follows up with residents, family and/or physician offices on lab results, including scheduling additional tests Coordinates referrals/appointments for patients and physician offices Obtains, carries out and follows physician orders for wound care. Participates in Department and QAPI meetings or activities when necessary. Ensures that equipment is maintained in clean and safe operating condition. Exercises independent judgment and decision-making. Communicates effectively with patients and their families, physicians and staff. Assists in assessing, admitting, discharging and transferring patients
