Job Description

Signature HealthCARE is seeking a compassionate Registered Nurse to provide resident-centered care in our long-term and post-acute communities. In this role, you will assess, plan, and coordinate individualized nursing care; administer medications and treatments; and lead the care team to promote dignity, safety, and optimal outcomes. You'll collaborate with an interdisciplinary team in a mission-driven, faith-based environment that values compassion, teamwork, and continuous improvement, with opportunities for ongoing training, professional growth, and meaningful impact on residents' lives. Responsibilities Deliver resident-centered nursing care in long-term and post-acute settings Assess, plan, implement, and evaluate individualized care plans Administer medications and treatments per provider orders and facility protocols Document assessments, interventions, and outcomes in EHR accurately and timely Collaborate with interdisciplinary team to optimize resident outcomes Supervise and support CNAs and other care team members Educate residents and families on care plans, treatments, and health management Ensure compliance with clinical standards, safety, and infection control policies Participate in quality improvement and resident satisfaction initiatives Support Signature Health CARE's mission, values, and faith-based culture Required Skills Patient assessment Care planning and coordination Medication administration IV therapy and wound care Electronic health records (EHR) documentation Infection prevention and control Clinical triage and critical thinking Interdisciplinary team collaboration Patient and family education Regulatory and compliance knowledge (long-term care)
