A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Case Manager is responsible on 24-hour basis for planning and managing comprehensive operations of specific clinical area or services such that they comply with philosophy, policies and procedures of Hospice and CVMC. Case Manager is responsible for ensuring delivery of competent, compassionate patient care through nursing process. Case Manager maintains appropriate human, material, technological and financial resources necessary for provision of patient care and serves as liaison between administration and employees. Individual demonstrates clinical expertise in assigned clinical areas, develops and maintains environment which promotes mutual trust, acceptance, recognition and respect; demonstrates personal and professional responsibility and assumes accountability for own professional development. Qualifications for the job... Graduate from an accredited school of nursing and currently licensed as a Registered Nurse in the State of Alabama. Preferred bachelor degree in Nursing or health related field. Valid Alabama driver's license, proof of current automobile liability insurance and reliable transportation required to carry out patient assignments. Three to four years' Hospice experience.
