Desired Qualifications:

Active Registered Nurse (RN) license Minimum of 1 year recent RN experience in home health settings Proven OASIS documentation skills (required) HomeCare HomeBase proficiency (strongly preferred) Reliable transportation and valid drivers license Competence in managing a full caseload independently

Primary Responsibilities:

Deliver comprehensive in-home nursing care and skilled interventions Accurately conduct and document patient assessments using OASIS Coordinate with physicians and interdisciplinary teams for customized care Administer wound care, medication, patient/family education, and chronic disease monitoring Effectively communicate patient status and needs to the clinical team
