Job Description

Make an impact providing care in your community as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity enables you to work across Ouachita County, ARincluding Camden, Stephens, and Beardenserving Medicare Advantage members in their homes. Each assessment visit averages 45 minutes and may range from annual health reviews to post-discharge follow-ups. Youll have the freedom to create your own schedule with flexible shifts available any day of the week and select a weekly or bi-weekly hourly commitment based on your availability.
Schedule:
Flexible shifts available Monday through Sunday Ability to create your own schedule Minimum weekly or bi-weekly hourly commitment
Responsibilities:
Perform comprehensive annual and post-discharge risk assessments for Medicare Advantage plan members Review and document prior/current medical history, ongoing conditions, and medications Educate patients about their health conditions and medications Identify gaps in patient care, making recommendations for improved health outcomes No prescribing of medications or treatment plan creation required
Qualifications:
Active Nurse Practitioner license in Arkansas Board certification through AANP or
ANCC MSN
degree from an accredited nursing school Current CPR (BLS) certification At least 6 months recent NP experience preferred This position offers competitive pay of $80-$90 per completed evaluation, as well as a mileage stipend and compensation for administrative time.
