A leading healthcare organization is seeking a dedicated Registered Nurse (RN) with proven experience in Home Health and OASIS documentation for a 13-week full-time contract. Schedule:

Full-time, Monday through Friday On-call rotation may be required

Key Responsibilities:

Deliver skilled in-home nursing care including assessments, interventions, and follow-ups Complete timely and accurate OASIS documentation for all patient visits Collaborate closely with physicians and interdisciplinary teams to formulate and execute patient-centered care plans Provide wound care, medication management, chronic disease monitoring, and patient/family education Communicate changes in patient condition and progress to the broader clinical team

Desired Qualifications:

Active, unencumbered RN license At least 1 year of recent RN experience in home health Proficiency with OASIS documentation (required) Experience with HomeCare HomeBase (strongly preferred) Reliable transportation and a valid drivers license
