We are seeking a dedicated Registered Nurse with proven home health experience, including OASIS documentation proficiency, for a full-time, 13-week contract assignment. This role is ideal for RNs confident in clinical skills, detail-oriented with documentation, and comfortable managing a structured schedulemaking a real difference in patients lives in the comfort of their own homes.

Schedule:

Full-time, Monday through Friday On-call duties may be assigned Daily visit expectations

Qualifications:

Active RN license At least one year of recent home health RN experience Required OASIS assessment and documentation experience Familiarity with HomeCare HomeBase (strongly preferred) Reliable transportation and valid drivers license Strong time management and patient communication skills

Responsibilities:

Deliver skilled nursing care in patients homes, including assessments, interventions, and education Complete accurate OASIS documentation in a timely manner Coordinate care and updates with physicians and the broader multidisciplinary team Manage wound care, administer medications, monitor disease processes, and educate patients and families Report changes in patient conditions promptly to the clinical team
