Job Description

Job Summary We are seeking a dedicated and detail-oriented MDS Coordinator/Assisted Director of Nursing to join our healthcare team. Must have an active LPN or RN license. In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs within long-term care and skilled nursing facilities. Your expertise will ensure accurate completion of Minimum Data Set (MDS) assessments, supporting compliance with federal and state regulations, including Medicare and Medicaid requirements. You will collaborate closely with interdisciplinary teams to optimize patient outcomes, streamline documentation processes, and enhance quality of care. Responsibilities Conduct thorough MDS assessments in accordance with clinical documentation standards and regulatory guidelines such as CMS (Centers for Medicare and Medicaid Services) policies. Review medical records, clinical documentation, and physician notes to ensure accuracy and completeness of data. Coordinate with nursing staff, physicians, case managers, and other healthcare professionals to gather necessary information for assessments. Ensure timely completion and submission of MDS schedules to support proper reimbursement and quality reporting. Maintain up-to-date knowledge of
ICD-10/ICD
coding, CPT coding, DRG (Diagnosis-Related Group) classifications, and long-term care regulations. Support utilization management efforts by reviewing documentation for appropriateness of services and level of care. Assist in compliance audits related to HEDIS (Healthcare Effectiveness Data and Information Set), NCQA standards, and other accreditation requirements. Skills Strong knowledge of PointClickCare. Proficiency in medical terminology, anatomy, physiology, and clinical documentation standards. Experience with
ICD-9/ICD-10
coding, CPT coding, DRG assignment, and Medicare regulations. Familiarity with managed care processes, utilization review, case management, and health information management. In-depth understanding of long-term care regulations, HIPAA compliance, hospital experience including ICU or Level I Trauma Center experience preferred. Excellent organizational skills with the ability to review complex medical records efficiently while maintaining attention to detail. Ability to work collaboratively within multidisciplinary teams in inpatient or outpatient settings including nursing homes or hospice care environments.
