Job Description

Deliver comprehensive care in a patient-focused setting by performing home health risk assessments as a Nurse Practitioner. In this rewarding per diem role, you'll visit Medicare Advantage members in Monticello as well as nearby communities such as Wilmar and Winchester in Drew County. The nature of each visit varies, including both annual health assessments and post-discharge follow-ups, typically lasting around 45 minutes.
Schedule:
Enjoy flexible shifts seven days a week Create your own work schedule Satisfy a minimum hourly commitment weekly or bi-weekly
Key Responsibilities:
Complete annual risk assessments for Medicare Advantage plan members in the comfort of their homes Assess patients medical history, current conditions, and medication usage Deliver clear education about health conditions and prescribed medications Identify and document any gaps in care; offer actionable recommendations to improve patient outcomes Please note: Treatment plan creation and medication prescribing are not part of this position
Qualifications:
Active Nurse Practitioner license in Arkansas Board Certification (AANP or ANCC) Current BLS/CPR certification At least six months of recent NP experience preferred Master of Science in Nursing (MSN) from an accredited program As part of this position, you will be compensated $80$90 per evaluation along with a mileage stipend and payment for administrative time.
