Job Description

As a Nurse Navigator for Orthopedics at Mercy, you will guide patients and families through the full continuum of orthopedic care, from referral and pre-op education to post-op recovery and rehabilitation. You will coordinate multidisciplinary care plans, ensure timely appointments, and assist with discharge planning and community resources. Collaborating closely with surgeons, therapists, and case managers, you will monitor patient progress, address barriers to care, and support quality, safety, and satisfaction initiatives while modeling Mercy's culture of compassion and respect. Responsibilities Coordinate the full continuum of orthopedic patient care from referral through post-op recovery. Provide patient and family education on diagnoses, procedures, and rehabilitation plans. Develop, implement, and adjust individualized care plans in collaboration with providers. Facilitate interdisciplinary communication among surgeons, nurses, therapists, and case managers. Monitor patient progress and address barriers to access, adherence, and recovery. Support discharge planning, including home care, equipment, and community resources. Use EHR systems to document care, track outcomes, and ensure care continuity. Participate in quality, safety, and patient experience improvement initiatives. Ensure appropriate utilization of services and alignment with clinical pathways.
