The Registered Nurse (RN) provides compassionate, patient-centered nursing care to individuals with serious or complex health needs. The RN assesses patient needs, supports symptom management, and coordinates care with patients, families, providers, and the interdisciplinary team. This role uses clinical judgment and palliative care experience to help align care with each patient's goals and preferences. The RN provides education, communicates changes in condition, and maintains accurate documentation.

THE REGISTERED NURSE WILL

Provide nursing assessments and identify changes in patient condition Support symptom management and implement nursing interventions within the RN scope of practice Coordinate individualized care with patients, families, providers, and other care team members Provide clear education and compassionate support to patients and families Document care accurately and communicate needs requiring timely follow-up

REQUIRED QUALIFICATIONS

Current, unrestricted Registered Nurse (RN) license in the state of practice Palliative care experience required Strong clinical assessment, communication, critical thinking, and care coordination skills Ability to work independently and collaboratively with an interdisciplinary team Proficiency with electronic health records and timely clinical documentation

PREFERRED QUALIFICATIONS

Experience caring for patients with serious or complex illness Palliative or hospice nursing certification

KEY RESPONSIBILITIES 1. CLINICAL ASSESSMENT AND NURSING CARE

Conduct and document comprehensive nursing assessments, including physical symptoms, functional needs, and psychosocial concerns Monitor changes in condition and communicate significant findings to the appropriate provider or care team member Implement nursing interventions and evaluate the patient's response within the RN scope of practice and established care plan 2.

PALLIATIVE CARE AND PATIENT SUPPORT

Apply palliative care principles to support comfort, quality of life, and patient-defined goals Assess symptoms and collaborate with the provider and care team on appropriate management strategies Support patients and families as they discuss care preferences, available services, and advance care planning 3.

CARE COORDINATION AND EDUCATION

Collaborate with providers and interdisciplinary team members to develop and update individualized plans of care Educate patients and families about the care plan, symptom reporting, medications, and follow-up needs Coordinate referrals and transitions of care as appropriate and communicate outstanding needs 4.

DOCUMENTATION AND PROFESSIONAL PRACTICE

Maintain accurate, timely documentation in the electronic health record Protect patient privacy and follow applicable professional standards and organizational procedures Participate in team meetings, education, and quality improvement activities as assigned
