Schedule & Hours:

Full-time, Monday through Friday On-call responsibilities are probable Structured daily visit expectations

Key Responsibilities:

Deliver comprehensive in-home nursing care and skilled clinical interventions Complete timely and accurate patient assessments and OASIS documentation Coordinate with physicians and interdisciplinary care teams to implement individualized care plans Administer wound care, medication management, disease monitoring, and patient/family education Effectively communicate changes in patient status to the clinical team

Qualifications:

Active RN license in good standing At least 1 year of recent, relevant home health RN experience Proficient with OASIS assessment and documentation Experience with HomeCare HomeBase software is strongly preferred Reliable transportation and a valid drivers license Ability to manage a structured, full-time schedule and consistent patient visits
