PRN LTAC RN

to deliver high-acuity care to medically complex patients in a long-term acute care setting. In this flexible, per diem role, you'll assess, monitor, and manage complex conditions, including ventilators, trachs, wounds, and IV therapies, while coordinating with an interdisciplinary team to ensure safe transitions of care. You'll document thoroughly, follow evidence-based protocols, and educate patients and families. Responsibilities Provide high-acuity bedside care to LTAC patients on a PRN basis Assess, plan, implement, and evaluate individualized care plans Administer medications, treatments, and manage complex lines and airways Monitor patient status, respond to changes, and escalate appropriately Collaborate with physicians and interdisciplinary teams on care plans and discharges Document care accurately in the EMR and maintain regulatory compliance Educate patients and families on conditions, treatments, and transitions of care Uphold infection control, safety, and quality standards in all clinical activities Required Skills LTAC nursing care High-acuity patient assessment Wound care management IV therapy and medication administration Ventilator and trach management Care planning and documentation (EMR) Infection prevention and control Patient and family education Interdisciplinary team collaboration Time management and prioritization
