Schedule:

Monday through Friday, full-time hours On-call responsibilities may be required Structured daily visit expectations

Key Responsibilities:

Deliver in-home nursing care, skilled interventions, and compassionate support for home health patients Complete thorough patient assessments and submit OASIS documentation accurately and promptly Collaborate closely with physicians and interdisciplinary care teams to develop and implement individualized care plans Perform wound care, medication management, disease monitoring, and patient/caregiver education Report clinical updates and significant patient status changes to the care team as needed

Qualifications:

Active, unrestricted RN license Minimum of 1 year of recent home health RN experience OASIS documentation proficiency required Experience with HomeCare HomeBase is strongly preferred Comfortable maintaining a structured, full-time schedule and meeting daily visit metrics Valid drivers license and reliable transportation
