Experience a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this role, you will visit Medicare Advantage plan members in Drew County, including Monticello, Wilmar, and Winchester. Each visit, typically lasting about 45 minutes, may involve either annual health assessments or post-discharge evaluations to ensure high-quality patient care directly in the home environment.

Key Responsibilities:
Conduct comprehensive home health risk assessments for Medicare Advantage plan participants Review and evaluate prior/current medical and medication history Educate patients on diagnoses, treatment plans, and medication management Identify potential gaps in care, documenting recommendations for improvement No need to develop treatment plans or prescribe medication
Qualifications:
Active Nurse Practitioner license in Arkansas Current BLS/CPR certification Board certification through AANP or ANCC At least 6 months of experience as a practicing Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution This position offers $80-$90 per evaluation, supplemented by a mileage stipend and compensation for administrative time.
