Job Description

Job Overview:
We are searching for multiple Public Health Nurses for a facility in Kayenta, AZ . The ideal candidates will possess a comprehensive understanding of public health principles, clinical nursing expertise, and experience working with diverse populations. As a Public Health Nurse, you will play a vital role in promoting health, preventing disease, and providing education within the community.
Type:
Contract Shift:
8a - 3p, M-F, no weekends
Pay Rate:
$46/hr local, $2025/wk travel rate
Experience:
RNs with primary care, public health, or ED experience would be helpful but not necessarily required.
Licensing:
Need AZ, UT, NM or a compact state license
Responsibilities:
Conduct comprehensive patient assessments to identify health needs and develop individualized care plans aligned with public health goals. Provide immunizations, screenings, and health education to diverse populations, including vulnerable groups such as individuals with developmental disabilities and seniors. Coordinate and facilitate community outreach programs focused on disease prevention, chronic disease management, and health promotion. Collaborate with healthcare providers, social services, and community organizations to ensure seamless care transitions and resource accessibility. Maintain accurate medical documentation in electronic health record (EHR) systems while ensuring compliance with HIPAA regulations. Offer triage services to evaluate urgent health concerns and determine appropriate levels of care or referrals. Participate in outbreak investigations, infection control practices, and emergency preparedness planning within the community setting.

From $2,025.00 per week
