The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while collaborating with a multidisciplinary team. Duties Conduct comprehensive assessments of patients' health status and needs in a home setting. Develop and implement individualized care plans based on patient assessments. Administer medications and treatments as prescribed, including injections and tube feeding. Provide specialized care in areas such as ostomy care, Wound Care and geriatrics. Monitor patients' progress and adjust care plans as necessary to ensure optimal outcomes. Educate patients and their families about health conditions, treatment options, and home care techniques. Maintain accurate documentation of patient care activities in compliance with regulatory standards. Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care. Stay informed about current best practices in nursing and healthcare regulations. Requirements Current Registered Nurse (RN) license in the state of practice. Experience in home health or community health nursing is preferred. Experience with medical-surgical nursing. Strong interpersonal skills to effectively communicate with patients, families, and healthcare teams. Ability to work independently while managing multiple patient cases effectively.
