Schedule & Expectations:

Monday Friday, 8:00 AM 5:00

PM Caseload:

30 points per week (typically 35 patient visits per day)

Field-based:

Daily travel to patient homes within an assigned territory (mileage reimbursement provided)

Essential Qualifications:

Active RN License in good standing within the state of practice Minimum of 1 year of recent Home Health experience (preferably with field visits) Proficient with Home Care Home Base (HCHB) electronic documentation Demonstrated competency with OASIS assessment tool Reliable transportation, valid driver's license, and auto insurance
