Life Care Centers of America seeks an MDS Nurse to lead resident assessment and care planning in our senior living and rehab community. In this role, you'll complete and manage MDS assessments, ensure accurate documentation, and support regulatory compliance while partnering with an interdisciplinary care team. You'll help drive quality outcomes, train staff on MDS processes, and communicate care plans with residents and families. Responsibilities Complete and manage MDS assessments in accordance with federal and state regulations Accurately document resident status, care needs, and treatment plans Collaborate with interdisciplinary team to develop and update care plans Review clinical records for accuracy and regulatory compliance Educate staff on MDS processes, documentation standards, and care planning Monitor quality indicators and participate in quality improvement initiatives Communicate with residents and families regarding care plans and progress Ensure timely submission of MDS data to appropriate agencies Support survey readiness and respond to surveyor requests Assist with staff orientation and ongoing clinical training related to MDS Required Skills Minimum Data Set (MDS) assessment Long-term care nursing Care planning Electronic medical records (EMR) Regulatory compliance (CMS, state) Clinical documentation Interdisciplinary team collaboration Resident assessment and evaluation Quality improvement in healthcare Geriatric and rehabilitation nursing
