charting experience is also required. Responsibilities Conduct thorough patient assessments to determine care needs, including vital signs, symptom management, and physiological evaluations. Develop and implement individualized care plans in collaboration with physicians, social workers, and family members to promote comfort and quality of life. Administer medications and treatments as prescribed, monitoring for effectiveness and adverse reactions. Provide emotional support and education to patients and families about disease progression, symptom management, and end-of-life options. Coordinate discharge planning, ensuring seamless transitions between inpatient, outpatient, or post-acute care settings. Maintain accurate medical records in compliance with HIPAA regulations and facility policies. Collaborate with interdisciplinary teams to optimize patient care outcomes and facilitate case management activities. Requirements Valid nursing license with current registration in the state of practice. Proven experience in critical care settings such as ICU, emergency medicine, or trauma centers; experience in hospital medicine or post-acute care is highly valued. Strong background in patient assessment, vital signs monitoring, medical terminology, and physiology knowledge. Familiarity with infectious disease care, behavioral health issues, geriatrics, pediatrics (PICU), or medical-surgical units enhances your ability to deliver comprehensive hospice care. Knowledge of HIPAA regulations and medical records management is essential for maintaining patient confidentiality. Excellent communication skills for patient service delivery and family education. Ability to work independently while collaborating effectively within multidisciplinary teams.
