Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Review and document prior/current medical history, chronic conditions, and medication use Educate members about their medical conditions and prescribed medications Spot gaps in care and recommend next steps to close those gaps Focus exclusively on assessmentno treatment planning or medication prescribing required Each assessment averages about 45 minutes

Qualifications:

Active, unrestricted Nurse Practitioner license in Arizona Board certified by AANP or

ANCC MSN

from an accredited nursing program Valid CPR (BLS) certification At least 6 months experience as a Nurse Practitioner strongly preferred In return for your expertise, youll receive $80-$90 per completed evaluation, including a mileage stipend and compensation for administrative time.
