Collaborate effectively with healthcare professionals across disciplines—including physicians, social workers, chaplains, and therapists—to ensure holistic patient support. Provide education to patients and families about hospice services, medication management, symptom control, and advance directives to empower informed decision-making. Utilize electronic health records (EHR) systems for accurate documentation of patient progress, medical records, discharge planning, and compliance with HIPAA regulations. Oversee discharge planning and transition processes from hospital or inpatient settings to home or hospice facilities, ensuring smooth continuity of care. Participate in ongoing quality improvement initiatives related to hospice care standards, infection control policies, and health regulation compliance. Qualifications Valid nursing license with current registration Experience with hospice, home health or outpatient clinics Strong knowledge of hospice, medical terminology, health regulation policies (including HIPAA), and electronic health records (EHR/EHR management). Proven ability to perform comprehensive patient assessments and manage complex clinical cases Excellent communication skills for client interaction and interdisciplinary collaboration; adept at managing patient service needs with compassion and professionalism. Familiarity with managed care processes, utilization management, discharge planning, diagnostic evaluation, and case management protocols.
