Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to develop tailored care plans. Manage complex wound care, including dressing changes and infection control measures, especially in patients with chronic or acute conditions. Provide education to patients and caregivers on disease management, medication administration, nutrition, and safety practices. Document all patient interactions accurately using Electronic Health Record (EHR) systems such as AXXESS and maintaining compliance with HIPAA and confidentiality policies. Coordinate with physicians, case managers, and other healthcare professionals for seamless continuity of care across settings like skilled nursing facilities or assisted living communities. Monitor patient progress through regular assessments and triage calls, adjusting care plans as needed to optimize health outcomes. Qualifications Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience in home health nursing preferred but not required or related settings such as hospice care, long-term care, or medical-surgical units. Strong knowledge of medical terminology, anatomy, physiology. Experience working with diverse patient populations including geriatrics, individuals with developmental disabilities, dementia or Alzheimer's disease. Ability to perform clinical procedures such as catheterization, tube feeding management, and sterile technique. Excellent communication skills to effectively collaborate with patients' families and multidisciplinary teams. Critical care experience preferred; additional skills in acute pain management, infection control, ventilator management or ICU experience are highly valued. Strong documentation skills to meet home health requirements and support compliance and quality assurance. Ability to manage an independent schedule and work autonomously, applying sound judgment, professional knowledge, and experience. This per diem position requires a minimum one-year commitment and a willingness to work under direct supervision while learning home health practices and compliance requirements.
