Looking for fulfilling patient interaction? Join our growing team at Community Home Health! We are hiring a per-diem Start of Care Registered Nurse to join our home health team . What we're looking for: California-licensed RN with or without home health experience. Must be a quick learner and be reliable. Valid BLS-CPR, reliable auto and auto insurance. Clean background check. Bilingual and/or wound care certified is a plus.
Responsibilities:
Provide skilled direct patient care in a home health setting. Assess and evaluate patient needs and develop individualized plans of care. Monitor patient progress and adjust care plans as needed. Obtain orders from physicians for any changes to care plans. Communicate effectively with office staff and other healthcare professional team members.
