Job Descriptions:

Assesses patient needs and develops plan of care with physician. Coordinates and oversees care plan with patient, family, and support staff—LVNs and nurse assistants. Monitors and evaluates patient response to treatment, medication, and healing or mobilization progress. Oversees case management. Assists with activities of daily living (ADLs)—mobility, bathing, grooming, toileting, etc. Administers medication and intravenous infusions. Takes vital signs, draws labs, and manages wound care. Works with clinical healthcare professionals to coordinate home care. Performs physical assessments.

Qualifications:

Graduate of an accredited school of nursing. One to two years of home health care experience. Current licensure in state and BLS/CPR certification. Bachelor's degree Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Ale to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements. Pay is determined per patient visit: Start of Care (SOC) $135 Recertification/ROC $120 Discharge (DC) $120
