Case Manager

LVN/RN Case Manager We are currently seeking a dedicated and organized LVN or RN Case Manager to join our team. This position is available as either part-time or full-time and requires in-office attendance.

Responsibilities include:

Coordinating and managing patient care plans Communicating with physicians, patients, and caregivers Reviewing and maintaining patient documentation and charts Scheduling visits and ensuring timely follow-ups Assisting with authorizations, referrals, and care coordination Monitoring compliance with healthcare regulations and company policies Collaborating with clinical and administrative staff to ensure quality patient care

Requirements:

Active California LVN or RN license Strong communication and organizational skills Ability to multitask in a fast-paced environment Previous case management or home health experience preferred Must be able to work in-office Bilingual candidates preferred

From $35.00 per hour
